Instructions for Registering Your New Patient Portal

 

The Wellness Company just made our 24-hour 7-day-a-week service even more convenient. Gain prompt access to a trusted medical provider, obtain prescriptions, access your fully-secured digital medical records, and access our continuity of care services. 

 

TO REGISTER ON THE WELLNESS COMPANY PATIENT PORTAL :

          1. STEP 1: Patient Portal Registration Email

            Once you have booked your appointment through The Wellness Company Virtual Care Portal, you will receive the following emails to the address you provided during the booking process:

            1. Appointment Confirmation email: This email contains the necessary information to join your virtual care appointment. Remember to use this email to access the appointment up to 30 minutes before the scheduled time.

            2. Patient Portal Registration: You will receive an email inviting you to join our Patient Portal account. Follow the link provided in the email to complete your registration. This step needs to be completed prior to your appointment time.

            3. Intake Form/ Questionnaire Notification: You will receive an email with a link to access the general intake form/questionnaire on your Patient Portal. This step needs to be completed prior to your appointment time.

    Locate your Patient Portal Registration email, which includes your Patient Portal Invite. This invite is necessary to begin the registration process. Click on the link provided in the email to initiate the registration. If clicking on the link does not work, you can manually copy and paste it into your web browser.

     
  1. STEP 3: Patient Portal Account Activation

    After clicking on the Patient Portal Invite link, you will be directed to the account activation page.

    To proceed, confirm your identity by entering your Date of Birth. Once entered, select "Next".

      1.   1. Create a password by entering it in the designated fields. Remember to enter it again to confirm and ensure that it meets the required criteria.
      2.   2. Enter the text displayed in the image box.
      3.   3. Check the box to confirm your agreement with the ChARM PHR Terms of Service. Then, select "Submit" to proceed.

       

STEP 5: Patient Portal Account Activation, Continued

  1.   1. An email will be sent to the email address associated with your account registration. This email will contain a One Time Password (OTP) that will be valid for 15 minutes.

  2.   2. Access your email inbox and locate the email with the OTP. Enter the OTP in the provided space. Select "Verify OTP" to complete your registration.

Your registration process is now complete.

    1. STEP 6: Accessing your Patient Portal Account

    To verify your account timezone, select the dropdown menu and choose the timezone that corresponds to your location. Then, click on "Update" to proceed to your Patient Portal homepage.

    1. STEP 7: Preparing for your Virtual Care Appoinment

  1. To prepare for your virtual care appointment, please ensure that you complete all questionnaires prior to your scheduled appointment time. Click on the "Questionnaire" icon to view any pending forms that need to be completed.

    1. STEP 8: Complete all Questionnaires / Intake Forms

    To ensure a smooth virtual care appointment, please complete all pending Questionnaire/Intake Forms labeled with the "New" status before your appointment time.

    Select each pending Questionnaire/Intake form and fill it out accordingly. Repeat this process for each form that is pending.

  1. Fill out the selected questionnaire/Intake Form to the best of your ability. Once you have completed the form, click on "Submit" to finalize your responses. Repeat this process for each form labeled with the "New" status.
